Add OHSA Incident


Only the authorized user is able to add new OHSA Incident data. To access this menu, click Menambah Karyawan icon on the index page of OHSA Incident then Add OHSA Incident page will appear as shown below.

Follow these steps to add OHSA Incident data.

  1. Company Name: Click pilih to choose the name of company whose OHSA Incident data will be added.
  2. Work Location: Click pilih icon to determine the work location.
  3. Location: Type in the location where the incident occured.
  4. Employee Name: Type in the name of employee involved in OHSA Incident. Move the selected name to the right box by clicking , or click  to remove the selection.
  5. OHSA Category : Click pilih to determine OHSA Category.
  6. Incident Time: Type in the date at Date field in calendar format, or click icon to choose the date. Input the time at Time field in HH:MM format.
  7. Description: Type in the description about the incident.
  8. Severity: Click pilih to determine severity level.
  9. Attachment: Upload the attachment file in .txt, .doc, .docx, .jpg, .png, or .gif format. This attachment can be the additional data related to the detail of incident.

After inputting the data, click  to save the data. Click  to cancel creating new data.
